There are numerous forms requested of students during their time at E. C. Glass.  Some of those forms have been placed on this site to assist you with completing the given tasks.  You can use this site to print and complete the needed forms and return them to the guidance counselor.  These forms are also available through Guidance.  In most instances, students receive the needed forms at the appropriate time.  Please browse through these and use at your discretion.

The guidance forms provided are available to be printed and used for your convenience. All students are required to have these forms when participating in the individual activities or academic programs. Students are given these forms at school, but you may access them through this site.  Please note that you will need to have Adobe Acrobat Reader to view these forms.  Acrobat Reader is available as a free download.
